Patient Registration
If you wish to register with Churchill Medical Centre you can download the following registration forms, fill them out and return them to the surgery. Alternatively, pop in to one of our surgeries and fill a form out when passing. Don’t worry if you do not have all the information available. Once you have registered you can see the doctor of your choice and can attend either of our surgeries – your electronic records will be available at both sites.
